Narrative Medical Records: The reports written by your doctor about your diagnosis, treatment and overall medical condition. These reports are filed with the Commission as a part of your case and provided to the insurer pursuant to the Rules of the Commission.
Notice: “Every injured employee or his representative shall immediately on the occurrence of an accident or soon thereafter as practicable, give or cause to be given to the employer a written notice of the accident. The notice shall state the name and address of the employee, the time and place of the accident, and the nature and cause of the accident and the injury.”
Please note: Just because you have given notice to your employer about your injury does not mean your claim is FILED with the Commission. That you must take care of yourself, preferably with the guidance of an experienced workers compensation attorney.
